    LR-2 Hayabusa.
    Nose 23057. Nose 057. Tail JG-3057. C/N FL-677.
    Hokubu Houmen Koukuutai, JGSDF, based at Okadama AP, Sapporo [City], Hokkaidou.

    11:23, took off, at Okadama AP, for medevac mission.
    Flight plan was land at 11:50 at Hakodate AP, Hokkaidou, and pick up doctor(s) and patient, and take off at 12:20, and land at 12:50 at Okadama AP.

    11:47, before landing at Hakodate AP, last radio contact with ATC.
    11:48, lost radar and radio contact, about 18 NM or 33 km west of Hakodate AP.

    Instrument flight during low visibility that already grounded fire department and police helicopters.

    From 12:45, JASDF and JMSDF aircraft began SAR near last location in JASDF radar.

    2017/05/16, dawn, fire department, police, soldiers, and civilian hunters mobilised for ground search.

    11:06, UH-60 of Akita Kyuunantai, JASDF, found wreck on slope, 3 km east of Hakamagoshiyama Mountain, upstream of Todagawa River, near Hokuto [City], Hokkaidou, 25 km west of Hakodate AP.

    Four crew killed, bodies recovered.

    Pilot: Captain Takamiyagi Isao.
    53 years old.

    Co-pilot: Major Yanagida Tomonori.
    41 years old.

    Aircraft mechanic: Staff Sergeant Okaya Takamasa.
    42 years old.

    Avionics mechanic: Sergeant Tamaki Hironobu.
    28 years old.

    JMOD announced accident aircraft normally had flight recorder and voice recorder, but on accident day, because electric system malfunctioned, flight recorder was not in aircraft.

    Last radio contact between ATC and pilot was maintain altitude 1,070 m, but last radar contact was altitude 900 m, and crash site was altitude 300 m.

    Patient was 50+ years old man at Hakodate Goryoukaku Hospital, and needed treatment at Sapporo Medical University Hospital.
    Distance was 300+ km, and by car, needed 4+ hours.
    18:20, patient arrived by ambulance.

    Last flight on 2017/05/13, from Nakashibetsu AP [Nakashibetsu Town, Hokkaidou] to Okadama AP, was normal.

    11:00, local condition at Hakodate AP, cloud ceiling was 500', and passed instrument flight condition minimum 200'.

    11:47, ATC instructed aircraft to maintain 3,500' for enter landing route, and pilot answered understood.

    Assigned to JMSDF Antarctic icebreaker AGB-5003 JDS Shirase.

    14:20, while descending, lost balance, and from 3-4 m altitude, crash-landed on its left side, at Iwakuni AB, Yamaguchi Prefecture.

    Four of eight crew lightly wounded.

    Witness said before accident, helicopter flew in circles, and practised hoisting down and up cargo.

    From April to October, JDS Shirase helicopters regularly trained at Iwakuni AB.

    2017 March, Kawaski license-built and delivered 13th and final MCH-101/CH-101 S/N 8660 to 51st Koukuutai, JMSDF.

    22:50, lost contact, sent distress signal, and crashed into sea, Tsugaru Strait, 90 km west-southwest of Tappizaki Cape, Aomori Prefecture.

    Four male crew.
    One crew rescued.
    Three crew missing.

    Pilot was Lieutenant Commander Satou Yuuki (36).

    DD-156 JDS Setogiri is Asagiri Class destroyer, of Escort Division 7, Escort Flotilla 3, JMSDF, based at JMSDF Oominato, Mutsu City, Aomori Prefecture.

    During accident, night landing and taking off training, with two other destroyers and two other helicopters.

    2017/09/07:
    JMSDF announced accident cause was human mistake, not mechanical malfunction.
    Evidence from data of flight recorder, voice recorder, and survivor.

    22:33, after took off, caution light warned values of azimuth indicator(s) at pilot and co-pilot's displays had big difference.

    22:48, main pilot switched off and on power supply of Attitude Heading Reference Systems [AHRS device that confirmed attitude of aircraft, and sent data to azimuth indicator], to correct error, according to control manual procedure.

    When AHRS power switched off and on, automatic controls that also received data from AHRS to maintain attitude of aircraft, became not stable.
    Nose pitched up, speed decreased, and attitude of aircraft began to lose balance.

    Height 90-120 m, main pilot attempted to correct attitude, and to accelerate.
    Nose pitched down, continued to descend, and crashed into sea, after more than 10 seconds.

    During the procedure, co-pilot and two crew should aware attitude of aircraft and situation of surrounding, but they did not.

    Same type helicopters would resume flights from 2017/09/08.

    Wednesday, 2017/09/13:
    JGSDF announced accident report.
    Evidence from data of flight recorder and voice recorder.

    During landing approach, pilots and Hakodate AP controller conversed normally.
    Pilot or co-pilot accidentally turned off auto-pilot, and aircraft began to descend.

    Radio communication button was on in side of control grips, auto-pilot button was on out side of control grips, and pilot or co-pilot may accidentally touched wrong button.
    30 seconds before crash, alarm [for two seconds] warned auto-pilot turned off, but pilots did not react.

    Aircraft turning right [away from destination], and from altitude 1,100 m, suddenly descended, but flying in clouds, pilots not aware of aircraft attitude.
    Soon, second alarm warned ground proximity.

    10 seconds after second alarm, aircraft crashed.
    Voice recorder recorded pilots' surprise just before crash.

    Six same type aircraft would resume flights from 2017/09/15.

    2017/09/22:
    JMSDF announced cause as pilots' mistakes.

    While hovering and hoisting down cargo, co-pilot increased engine output too much.
    Pilot used another lever to decrease engine output.

    Co-pilot not aware of pilot's action, and continued to increase engine output.
    Helicopter vertically vibrated, and after crash-landed, continued to bounce up and down, lost balance, and rolled onto side.

    Pilot and co-pilot at same time did different actions were human mistake, not mechanical failure.

    13 same type helicopters would resume flights from tomorrow.

    During inspection, in ammunition depot, several air to air missiles for F-15, were discovered as damaged.
    Repair would cost 29.5 million Yen.

    2017/09/22:
    JASDF announced cause as trailers that carried the missiles, had collided.
    JASDF would investigate who were responsible for the damage, and why no one reported the damage.

    16:36, took off, at JGSDF Metabaru.
    After maintenance, test flight, from JGSDF Metabaru, to Kurume City, Fukuoka Prefecture, then to Asakura City, Fukuoka Prefecture, then return to base.

    During accident, only 11 years old girl at first house, lightly wounded leg.

    Engine trouble, and returning to base, for emergency landing.
    After radio contact lost, JGSDF Metabaru control tower witnessed helicopter flew from east to west, and crashed into ground, nose down.

    Local weather was cold, dry, sometimes snowing.

    Thats great, and please don't get me wrong as I am not trying to close it down.

    Maybe it should be a "sticky" or some other form of archive instead of discussion topic.

    Imagine if we did same thread here for many other airforces. It would be too many non discussion topics with lists of accidents one for each country.

    First, each month, Air Forces Monthly mag has 2-3 pages that report very recent military aviation accidents.
    I send to AFM editors, such accident reports for military aviation accidents in China, Taiwan, and maybe elsewhere in Northeast Asia.

    In AFM, the accident summaries tend to be brief, as the editors understandably cropped out a lot of juicy details, so I also post/share my accident reports in this forum.

    Second, back in the 1960s, two Thai military aircraft crashed in Taiwan.
    In 2010, I posted in this forum, very concise details about the two accidents, and didn't follow up them.

    Coincidentally, on the day this JGSDF AH-64 crashed, a relative of one of the victims of one of the Thai military aircraft crashes, read my old (2010) post, and asked if I knew more about that crash.
    For 50+ years, apparently the victim's children and relatives have been uncertain about how and where in Taiwan did the victim's aircraft crash.

    They thought it crashed into a mountain, but couldn't find/match the mountain name.
    AFAIK from on-line sources (mostly in English) I previously read, it was recorded as crashed into sea near Hsin Chu (either Hsin Chu City (coastal) or Hsin Chu County (wide)); but a contemporary military document hinted it crashed on ground.

    To satisfy my own curiosity, I again (2018) on-line researched (in Chinese and English) that crash, and found new details, such as contemporary news photos with captions, that confirmed it crashed into a mountain near a named town/village in Hsin Chu County.

    Wednesday, 2018/02/14:
    JMOD announced accident cause.
    Pilots misunderstood altitude.

    Night flight, helicopter flew at 300 m, and began descend to avoid clouds.
    Vertical speed indicator took some seconds to display actual rate of descend.

    Pilots read lagging indicator, and misunderstood rate of descend.
    Pilots not aware descend at two times normal rate, and after 45 seconds, crashed into sea.

    Two pilots wore night vision devices, but without moonlight, pilots and crew not aware altitude and situation.

    At 75 m, low altitude alarm sounded, recorded by voice recorder, but four crew did not react.
    After six seconds, crashed into sea.
