    15 April, 17:05, JDS Nagashima found wreck on sea bottom, tens of metres north of crash site, 50 m below surface.
    Robot with camera, confirmed fuselage serial number.
    16 April, 6:00, divers discovered pilot's body, one to two metres from wreck.
    Pilot's life vest not inflated.

    16 April, 7:35, JDS Matsuyuki arrived at JMSDF Oominato for investigation.

    During flight, Lieutenant Junior Grade Sakaiwaki at left pilot seat.
    Lieutenant Commander Miyanaga at right pilot seat.

    2012.04.20:
    Helicopter recovered by civilian salvage ship, and delivered to hangar at JMSDF Oominato.
    Analyse flight data recorder for one to two months.

    On JDS Matsuyuki, damage was 15 m above sea surface.
    Helicopter height was 5 m.
    Helicopter was altitude 10 m above sea surface.

    On April 19, Lieutenant Commander Miyanaga's funeral ceremony at base, then delivered to actual home in Miyazaki City.

    401st Hikoutai, 1st Yusou Koukuutai, JASDF, based at Komaki AB, Aichi Prefecture.
    13:45, above Enshuunada Sea, during trianing with UH-60 SAR helicopter, left refuelling pod below left wing, malfunctioned, and could not retract refuelling hose.
    Diameter, eight cm. Length, 30 metres.

    15:10, at 330 metres above sea, dumped refuelling hose into sea, 65 km south of Hamamatsu City, after other helicopters and transport aircraft assured no ship.

    16:00, returned to base.

    18:20, took off.
    Above Pacific Ocean, trained.
    After 45 minutes, landed.
    19:20, ground crew didn't find components.

    During training flight, above coast of central Akita Prefecture, dropped component.

    9:00, before morning training flight, component present.
    17:10, before night training flight, component missing.

    Component is cushion material to protect fuselage when storing aircraft collision avoidance light.

    Rubber cover.
    Length, 2 cm.
    Weight, 3 g.
    Width, 4.3 cm.

    Four crew.

    JA 6812.
    Akita Prefectural Police.
    During rescue mission, tail rotor hit tree branches.
    Aborted mission, and returned to Akita AP.

    Four crew not wounded.

    17:10, took off, to rescue eight hikers endangered by flooded swamp, on Moriyoshi Mountain [1,454 metres], Kita Akita City.
    17:35, 30 m above hikers, during hover, air flow caused helicopter to descend four to five metres, and hit tree branches.

    Three hikers rescued by Prefectural Fire Department helicopter in same mission.
    Five hikers by themselves descended mountain.

    3rd Koukuutai, 4th Koukuugun, JMSDF, based at Atsugi JMSDF AB, Kanagawa Prefecture.

    Four crew members of JMSDF P-3C.
    Pilot 1: Chief Petty Officer (23).
    Pilot 2: Chief Petty Officer (24).
    Communications operator 1: Chief Petty Officer (23).
    Communications operator 2: Petty Officer First Class (23).

    Drank, drove, and fell off car.
    On 2012.08.27, convicted and sentenced for 18 to 30 days of suspension.

    Between 20:00 and 12:00 on 20 June 2012, ate and drank beer, cocktail, and wine, at Yamato City, Kanagawa Prefecture, and walked back to base.
    In base, to return to dormitory, communications operator 1 (23) drove four-wheel-drive car.
    Pilot 1 (23) sat at passenger's seat.
    Communications operator 2 (23) and pilot 2 (24) stepped on rear bumper, and held onto car.

    Driver knew the two were on rear bumper, and drove at 20 km/h.
    After 100 m, the two fell off.
    Ambulance took them to Yamato City hospital.
    Heavily wounded: bleeding in the brain, and skull fracture.
    Hospitalised until middle July.

    JMSDF final accident report.
    Cause was pilot's judgement mistake.
    Lieutenant Commander [posthumous Commander] Miyanaga Masahiko.

    During low pass to farewell JDS Matsuyuki, flew too close to ship.
    Co-pilot and other crew members did not warn pilot, was also cause.

    During training at exercise area near Aonogahara JGSDF Base, dropped component.

    Part of aluminum part of azimuth pointer to check position of camera at nose.
    Diameter, 8 mm.
    Length, 15 cm.
    Weight, 10 g.

    16:40, after completed training at Aonogahara exercise area, took off, for Yao JGSDF Base, Yao City, Oosaka Prefecture.
    After five minutes, near Katou City, Hyougo Prefecture, pilot heard sound of something hit nose, and returned to exercise area.

    20:00, during landing, could not keep heading, and right landing gear wheel broke one runway right-side light and one taxiway light.
    Runway closed about 40 minutes for cleaning.

    Two pilots.
    First Lieutenants.

    16 civilian flights delayed: 10 departures and six arrivals.
    Departures delayed 42 minutes, and arrivals delayed 39 minutes.
    One arrival flight from Yonaguni AP, RAC 880, diverted to Miyako AP.

    18:15, at Naha AB, took off, for night training.

    After landing, five rivet heads missing from exterior panel at aircraft nose.

    11:45, three km southwest of air base, 400 m above Ayase water purification facility, bird strike.
    Aborted training, and returned to base.

    204th Hikoutai, 83rd Koukuutai, JASDF, based at Naha AB, Okinawa Prefecture.

    After scramble mission, after 13:00, landed at Naha AB, left rear tire punctured, and stopped on runway.
    After 30 minutes, towed away from runway.
    One of flight of two.

    Runway closed one hour, or until 14:05.
    15 civilian flights delayed or diverted.
    Seven departure flights [from Haneda, Hiroshima, Takamatsu, others] delayed three hours.
    Eight arrival flights diverted to Ishikaki Island [Okinawa Prefecture], Kadena AB [Okinawa Prefecture], Kagoshima [Kagoshima Prefecture].
    Three of these eight flights diverted to Kadena AB.
    By 17:30, these eight flights arrived at Naha AP.
    Also, two flights from Ishikaki Island and Kumamoto [Kumamoto Prefecture] returned to departure airports, and cancelled.

    JASDF, based at Iruma AB, Dasaitama Prefecture.
    11:05, at Komatsu AB, Ishikawa Prefecture, during landing, left and right tires punctured, fuselage contacted ground, and stopped at middle of 2,700 m runway.

    Two pilots not wounded.

    Colonel (42).
    Flight time was 2,300 hours.

    Lieutenant Colonel (39).

    Runway closed until 11:35.
    IBEX Airlines flight 17 from Narita AP, diverted to Itami AB.
    JAL flight 1275 from Haneda AP, delayed 38 minutes.

    10:00, took off, at Iruma AB, for adjust radar and other tasks.

    Accident cause investigating.
    High probability of pilots' inappropriate brake operation, or brake malfunction, caused sudden overpressure, and diameter 55 cm tires punctured.

    During last five years, T-4 landed and punctured tires, happened three times.
    None at Komatsu AB.
    Both left and right tires punctured, did not happen.
