204th Hikoutai, 83rd Koukuutai, JASDF, based at Naha AB, Okinawa Prefecture.

9:59, took off, at Naha AB. Leader of flight of four, and not armed.

10:33, during air combat training, above East China Sea, 180 km northwest of Naha City, pilot declared emergency.
140 km northwest of Iejima Island lighthouse.

10:35, disappeared from radar, and crashed into sea.
10:46, in crash area, wingmen spotted debris.

11:12 and after, JCG [11th Region] spotted tail fin.
11:39 and after, spotted part of horizontal stabiliser, speed brake, and other debris.

Pilot was Major Kawakubo Yuuji (37).
Instructor pilot, and has total 3,000 hours. 1,700 hours with F-15.

Aircraft accepted in 1987, and has no accident history.
Ground display at America Fest 2011, at Kadena AB, Okinawa Prefecture, on 1-2 July 2011.

JDS Haruyuki recovered tail fin, and confirmed serial number.

JASDF announced a fisherman claimed to have witnessed the accident F-15 flew nose-first into the ocean, and it had no fire or smoke, so it probably didn't disintegrate or explode in the air.

JASDF confirmed accident F-15 debris found by man [who lives in Nara City] at coast, Shirahama Town, Wakayama Prefecture, 1,080 km from accident area.

Debris was panel from front edge of left horizontal stabiliser.
Length, 30-40 cm.
Thick, 6 cm.
Width, 60 cm.
"No step" in Japanese, printed on panel.

Pilot still missing.

JASDF announced civilian salvage ship discovered tens of pieces of debris on oceanbed, on 10 August.
Included parts of engines and landing gear.
Pilot and flight recorder were not discovered.
Will attempt to recover debris.

11:35, F-4 took off.
11:45, 5,800 m above Hida, Gifu Prefecture, wingman saw fuel leak as mist from fuselage, and returned to base.

8:45, after training, during landing, 500 m above Yamaguchi Town and Negami Town, both of Noumi City, four km north-northwest of Komatsu AB, dropped one empty drop tank, and two fins from back end of inert training missile.

Pilot, Major not named (43), claimed, during landing, heard noise, and in back mirror, saw fire behind F-15.

Drop tank below left wing. One of three drop tanks.
Diameter, 80 cm. Length, 6.6 m. Weight, 155 kg.
Explosives in pylon, to jettison drop tank, did not explode.
Fragments found near sewage facility "Ishikawa Prefecture Midorigaoka purification centre".

8:06, took off, with three other F-15.

1998 October, accident aircraft constructed.
2011 January, assigned to Komatsu AB.

2011.10.09:
Possible accident cause is: drop tank interior over-pressurised, and exploded.
Normally, when fuel exited drop tank, air entered drop tank, to keep pressure.
When over-pressured, pressure escaped from valve.

At Komatsu AB, all grounded F-15 checked for system that, to adjust pressure, delivered air to and fuel from drop tank.
Also, electronic system that displayed in cockpit, amount of fuel in drop tank.

JASDF announced intermediate report.
Accident F-15 checked, and had no abnormal.
Accident cause possibly drop tank or pylon below wing.
After examined debris, high chance of drop tank cracked during flight.
Inside drop tank, air pressure increased to very high, static electricity and wiring short-circuited, detonated fuel, and exploded.

During 40 minutes training flight, right wing dropped aluminum part.

Preload pad. Buffer material between drop tank pylon and right wing.
Diameter, 7 cm. Thick, 0.2 mm. Weight, 6.4 g.

9:56, took off.
Trained above sea, near Shikoku Island.
10:39, landed.
During after-flight check, part reported missing.

From 31 October, all F-15 flights resumed, without drop tanks.

JASDF announced accident cause was probably pilot was partly conscious, or unconscious.
According to flight recorder, at altitude 3,200 m, turned right, and descended.
Condition was above 5.5 G, and continued eight seconds.

Pilot stopped turning.
At 5,000', decelerated, and pitched up towards horizontal two times.
10 seconds after first attempt, resumed to descend.
Down to crash, wings were parallel to ocean surface.
Probability of vertigo was low or no.
Pilot lost control and feeling of altitude.

JASDF announced accident report.
In drop tank, nut that fastened metal pipe that contained cables was missing.
Cables were part of fuel delivery electromagnetic valve and fuel sensor.
Vibrations damaged cable vinyl insulation, and exposed cable metal wires.
Metal wires and metal part of drop tank short-circuited, and detonated fuel gas in drop tank.
JASDF checked other drop tanks, and 20% had loose nuts.
Old maintenance procedures did not check cable nuts in drop tanks.
Accident drop tank was not checked after 2006.
New procedures open drop tanks, and check cables and nuts, once a year.

12:30, aborted take off, did not enter taxiway, and departed east edge of runway.
Stopped on grass, 10 m beyond runway.
Second of two F-15 for emergency scramble against intruder.

Pilot not wounded.
First Lieutenant (28).

From 12:31, Naha AP closed.
After 13:00, two ANA flights diverted to Kadena AB, Okinawa Prefecture. Other flights diverted to Ishigaki, Kagoshima, and Miyako.
62 flights and 5,903 passengers of six domestic and foreign airlines cancelled, delayed, or diverted.
13 flights of four domestic airlines were diverted.

Two F-15 scrambled against Chinese aircraft.
When second F-15 was taking off, civilian aircraft was landing.
Controller commanded both aircraft to abort landing and take off.
Second F-15 ran onto grass.
Controller allowed two F-15 to take off before civilian aircraft landed, but decided civilian aircraft and second F-15 were too close.
Informant claimed pilot shut down engines, did not brake, and lost control.

On December 20, Naha AB commander and senior officers apologised to Okinawa Prefecture deputy governor.

During training, dropped distress signal tubes, into Ise City.
Set of two metal cylinders, contained explosive, in cloth bag, carried by pilot (37, Major) and co-pilot (34).
Diameter, 5 cm. Length, 14 cm. Weight, 550 g.

January 12, from 16:30, training flight to Dainichiyama Mountain, Watarai Town, Mie Prefecture. Two hours. Total 26 km.

January 13, after 10:00, recovered from slate roof of factory, 500 m to 700 m north of JGSDF base.
Factory worker reported he heard sound of thing dropped on roof, on January 12 evening.

School claimed pilot and co-pilot brought the tubes.
When they boarded helicopter, they possibly placed them near the steps on helicopter, forgot them, and took off.

During flight, dropped metal wire antenna, and rubber component and screw that attached antenna.
Diameter, 1 mm. Length, 2 m. Weight, 100 g.

During flight from 13:20 to 15:48, flew above Higashiyamato City, Kodaira City, Kokubunji City, Fuchuu City, Kokuritsu City, Tama City, and Hino City.

Higashiyamato in Tokyo, Kodaira, Kokubunji, Fuchu, National City, Tama, Hino jumped over in order, that are missing parts, as when landing on the airport around 48 minutes at 3:00 pm I noticed that.

During checks before flight scheduled at 13:00, could not find pin.

9:15, 60 metres northwest of runway middle, crashed, and inverted.
Four main rotor blades detached.
No fire or oil leak.

One of three crew, Chief Petty Officer Hasegawa Ken'ichi (49) was lightly wounded.
Not pilot or co-pilot.

Pilot was Lieutenant Commander Sekikawa Satoshi (50).
Co-pilot was Petty Officer First Class Hayashi Tomohiro (24).

Bad visibility because snow.
Wind speed was 13 metres.
During air taxiing training and low-altitude flight above runway, controller and helicopter lost radio contact.
Soldier went to runway to confirm helicopter, and found helicopter inverted.
