    has a list of ROCAF AT-3, F-5, F-16, Mirage 2000 accidents, 1990-2003.

    2003.09.27, Ching Kuo IDF, serial number 1625. During training sortie, crashed into sea. Lieutenant-Colonel WANG Pei Jiang and First Lieutenant CAI Kun Nan ejected and saved.

    2003.06.11, AT-3 trainer, serial number 0827. During training sortie, crashed into mountain. Major DUAN Lian Sheng and QIU Yi Feng martyred.
    (I'm uncertain about Qiu's rank.)

    2002.11.14, Mirage 2000, serial number 2058. During training sortie, crashed into sea. Lieutenant-Colonel ZHANG Jian Xiang and First Lieutenant LIU Hao Fan martyred.

    2002.04.29, Ching Kuo IDF, serial number 1617, crashed.
    However, the (IMO more reliable) list in Taiwan Air Power reported this happened in 2003.

    2001.10.05, afternoon, F-5F, serial number 5356. During training sortie, crashed. Major LI Zhi Min and Captain KANG Hong Zhou martyred.

    1999.01.25, F-16, serial number 6813. During training sortie, crashed. Lieutenant-Colonel ZHANG Jia Kun and Major DAI Wei Kang martyred.

    1998.09.04, AT-3, serial number 0826, caught fire on runway. One pilot saved.

    1997.06.27, AT-3, serial number 0833, of the Thunder Tiger aerobatic team, crashed onto runway. Two pilots martyred.
    However, the list in Taiwan Air Power reported one pilot was martyred.

    1996.02.28, AT-3, serial number 0843, crashed. One pilot saved.

    1995.06.12, AT-3, serial number 0805, accident on runway. Two pilots saved.

    1994.11.07, AT-3, serial number 0858, crashed into sea. Two pilots saved.

    1994.01.26, AT-3, serial number 0850, crashed into sea. One pilot saved, one pilot martyred.

    1992.10.14, AT-3, serial number 0804, crashed into mountain. Two pilots martyred.

    1991.04.23, AT-3, serial number 0831, crashed. Pilot saved.
    However, the list in Taiwan Air Power reported two pilots saved.

    1990.09.06, AT-3, serial number 0837, crashed. Pilot saved.

    some of which have a photo, reported:

    On 24 June 2005, a ROCAF F-16B, number 6812, had a landing accident at Chia Yi AB, Chia Yi Hsien (Chia Yi County), Taiwan.

    At 09:15, it took off for a training sortie over the ocean.
    At 10:12, it came back and landed for 4,000 feet on the runway, which was wet with rain. It couldn't brake in time, and ran off runway, into the grass.
    The aircraft and crew weren't damaged or wounded.
    It was towed away for inspection.
    The ROCAF asked the AP to remain closed until 14:30.

    Hello Don,

    I'm intrigued by your use of the word "martyred".
    I did't know the Chinese used that word.
    It amazes me that someone who works in the PRC uses this word to describe the fate of RoC pilots.

    Both PRC and ROC used the word "martyred". You can find them in documents from way back describing people KIA as martyred, even before WWII.
    In Chinese it is 牺牲 and literally means (ultimate) sacrifice.

    sounds like sacrifice sounds closer. Maybe Don Chan is a Chinese Muslim. There are some of them.

    On 19 October 2005, at 19:18, a ROCN S-70CM2 ASW helo, serial number 2316, crashed into the sea, near Zuo Ying, Kao Hsiung City, Taiwan, during a landing-on-warship exercise with the ROCN Cheung Kung Class frigate FFG-1107 ROCS Tzu I.

    The helo belonged to 701st Squadron, normally based at Hua Lien, now temporarily at Zuo Ying.
    Three crew. Co-pilot and instructor Lieutenant-Commander YANG Shun Yuan killed, body recovered at 19:54. ASW officer HUANG and pilot Lieutenant SONG missing. Yang had 2,700 flight hours, Huang 1,706, and Song 1,070.

    For Song to pass the night landing-on-warship exercise, the helo was tasked with landing on the frigate three times. The first and second landings were successful. During the third landing, its angle was too high and speed too fast, so it climbed and attempted again, but in 10 seconds, it suddenly descended from altitude 80 ft and crashed, while witnessed by the frigate crew.
    SAR effort by at least three heloes and 13 ships.
    By 20:30, debris and oil slick found.

    Also, on 12 January 1994, a ROCN S-70CM1 ASW helo, serial number 2311, crashed into sea, near Hua Lien, after took off from a Cheung Kung Class frigate and stalled.
    Fell from altitude nine m.
    One killed.

    Morning.
    26th FS, 401st FW, ROCAF.
    Took off at Jia Shan AB, Hua Lien, Taiwan.
    One of two F-16s training in eastern air space.
    At 10:02, generator warning light lit up.
    About 10:22, landed at Ching Chuan Kang AB, Tai Chung, Taiwan.
    Pilot XU Hong Zhou not wounded.
    Other F-16 = a F-16B also landed there.
    Ching Chuan Kang AB reportedly operated only Ching Kuo IDF, and had no F-16 ground crew. F-16 ground crew summoned from Hua Lien to repair.

    UH-1H. 355.
    601st Air Cavalry Brigade, Army Aviation and Special Force Command, ROCA, based at Long Tan, Tao Yuan County.
    16:28, during aerial recon training sortie, collided with a police broadcast tower on Qi Shan Mountain (Qi Shan, Zhong Liao 2 Lu, 100 Hao), and crashed into a forest, in Zhong Liao Shan area, near Qi Shan Town, Gao Xiong County, southern Taiwan.
    Reportedly carried eight crew, five of whom killed, three missing.
    Killed included: brigade commander Colonel Chen, assault battalion commander Lieutenant Colonel Ma, battalion chief of staff Lieutenant Colonel Lai, co-pilot Captain Wu, mechanic NCO [Sergeant Major, or Master Sergeant, or Sergeant First Class] Luo.
    Missing included: deputy brigade commander Colonel Lin, pilot Major Liu, company commander Major Zhu.

    Police broadcast frequency FM 104.9 disrupted in accident area.
    Local witnesses claimed weather was raining and very foggy.
    Helicopter broke into three parts, with the tail hanging on the tower.

    15:52, took off at Gui Ren AB, Tainan County.

    737th Wing, ROCAF.
    9:39, during low-altitude air-to-mud training, while climbing, crashed into a building, armoured battalion depot, Hu Kou, Xin Zhu.
    Two pilots killed. Pilot Major Wei, co-pilot Captain Zhan.
    Bodies recovered and delivered to ROCA #804 Hospital.

    Crash site caught fire.
    On ground, two Singaporean soldiers killed, two Singaporean soldiers wounded.

    9:00, took off at Zhi Hang AB, Tai Dong.

    11 May 2007:
    During the Han Kuang 23 exercise in Taiwan, after a ROCAF F-5F crashed into a Singaporean Army soldiers' building, two of the burnt soldiers were immediately flown back to Singapore.
    One of the two, of Chinese ancestry, passed away in May. 8(

    The other, of non-Chinese ancestry, was at last discharged from his hospital on 8 July. 8)
    He claimed he was fetching something when suddenly he heard a big noise and everything about him caught fire. He ran out of the building, and his comrades rescued him. Then, he was in a coma for six weeks, and 45% burnt. 8(

    25 January 1999:
    F-16B. 6813.
    22nd TFS, ROCAF, based at Chia Yi AB, and deployed to Hua Lien AB.
    Crashed into Jin Zhen Mountain, in Tai Ma Li Mountain area, Tai Tung County.
    Two pilots killed. Pilot Major Dai Wei Kang, co-pilot Lieutenant Colonel Chang Chia Kun.

    At 16:15, it took off at Hua Lien AB, and flew to Tai Tung air-space, for instrument flight training.
    At 16:40, it began TACAN training, 23 klicks from Tai Tung AB, and searching for magnetic track R-210.
    Seven minutes later, or reportedly 16:54, it disppeared from radar screens, in direction of magnetic track R-232 from beacon.
    At 17:03, Chih Hang AB scrambled S-70C SAR helo.
    At 17:40, helo reported fire and explosions in Tai Ma Li Mountain area.
    At 20:30, personnel on ground reached the wreck, in a tea farm, in Bei Li Village, 860 m ASL.
    Tea factory caught fire, but no civilian wounded.

    On previous day, Dai flew same course and training.
    Just before accident, while descending with TACAN, the aircraft should turn right 30 degrees for landing, but didn't, at a spot about 3-5 NM from coast.
    ATC advised the pilots, but after 48 seconds, the aircraft crashed into Jin Zhen Mountain.
    Local weather was clouds and light rain from 1,500' AGL, or 4,000' ASL.

    Chang (43 or 45?) had 3,000 flight hours.
    Dai (30) had 1,300 flight hours.
    Dai's wife's younger sister's boyfriend [or fiance?], Zhang Jia Cheng, was also a ROCAF F-16 pilot who crashed in the previous year, on 20 March 1998.

    14:30, missing, about 50 NM southwest of Peng Hu.
    Latitude north 23 degrees 37 minutes; longitude east 118 degrees 53 minutes.
    Two pilots missing, bodies not recovered, assumed killed. Pilot Captain Zhang Jia Cheng (28), co-pilot Major Wen (34).

    First ROCAF F-16 crash.

    13:58, took off at Chia Yi AB.
    During training, crashed into sea, west of Hua Yu, Peng Hu County.

    Wen had 1,661 flight hours, including 220 hours with F-16.
    Zhang had 975 flight hours, including 108 hours with F-16.

    reported: soon, the marking and shape of one piece of debris appeared as a Mk 20 cluster bomb shell. A bombing range is 20 klicks from Hua Yu, so the debris may not be F-16.
